When you are told to hit your balls you already know it will hurt, so when you hit you end up pulling your punches, you do not hit as hard as you should. Who wants to make themselves feed bad on purpose?! This causes a bit of a problem though, doesn't it? When I tell you to smack your balls around I expect you to smack your balls around with the same vigor that I would if I were doing it to you! When I say to hit them then I want you to HIT THEM HARD.
You overthink when it comes to CBT. You know how it will feel when you hit them hard and how it will feel later on so you don't hit as hard as I want. What if I could help you fix that? What if you could hit them as hard as I would want you to and you'd feel almost nothing? Then maybe you can start to get over this fear of smacking your balls around how I want you to!
You can learn to hit yourself properly while not having to worry about how this is going to feel until later on. When you can get past the initial shock of how scary and painful this can be I think we can really start to have some fun!
